PILOMATRICAL
CARCINOMA
Pilomatrical carcinoma (matrical carcinoma) is a rare tumor found on the head and neck of older adults. Clinical appearances are generally not distinctive. Most cases probably develop de novo, but malignant transformation from a benign pilomatricoma has been reported. Histopathologic examination shows irregularly sized and variously shaped cutaneous/subcutaneous epithelial islands of basaloid (matrical and supramatrical) cells with conspicuous foci of shadow cells . The basaloid cells reveal nuclear atypia and mitoses, as well as areas of geographic necrosis.Pilomatrical carcinoma is primarily locally aggressive, but distant metastases, especially to lungs, bones, and lymphatics, have rarely been reported. Treatment of choice is surgical excision with adequate margins
